Effective July 1,2003, the Offence Act Forms Regulation, B.C. Reg. 422/90, is amended
(a)  by adding the following section: 
Statement and written reasons
The statement under section 15.4 (1) (a) and written reasons under section 15.4 (1) (b) of the Act must be in Form M.
(b)  by repealing Form G and substituting the attached Form G,
(c)  by repealing Form J & substituting the attached Form J, and
(d)  by adding the attached Form M.

Statutory Authority: Child Care Subsidy

Section 4 of the Child Care Subsidy Regulation, B,C. Reg, 74/97, is amended by adding the following subsections:
(3)  A parent ceases to be eligible for a child care subsidy on the date that is 6 months after the date of application under subsection (1) or (4), as applicable, unless, before that date, the parent provides to the minister confirmation of the parent's continuing eligibility and need for the child care subsidy.
(4)  A parent ceases to be eligible for a child care subsidy on the date that is 12 months after the date of application under subsection (1) or this subsection, as applicable, unless, before that date, the parent completes an application referred to in subsection (1) and otherwise complies with that subsection.
Schedule A is amended
(a)   in each table by striking out "Four Hours or Less Daily" and substituting "Four Hours or Less Daily unless Both Before and After School Care Provided",
(b)   in each table by striking out "More Than Four Hours Daily" and substituting "More Than Four Hours Daily or Both Before and After School Care Provided", and
(c)   in the table entitled "Subsidy Rates for In-Own-Home Settings" by striking out "In-own-Home" wherever it appears and substituting "In-Child's-Home".
