The appointment of Janet Baltes as Assistant Deputy Minister, Ministry of Small Business and Revenue, made by OIC 20/2007, is rescinded and she is appointed Assistant Deputy Minister, Strategic Services and Tax Policy, Ministry of Finance.

Statutory Authority: E-Health (Personal Health Information Access and Protection of Privacy), s. 3

1.  The Provincial Laboratory Information Solution (PLIS) Repository, as a central repository of laboratory tests, is designated as a health information bank.

2.  The types of personal health information contained are identified in Appendix A.

3.  The purposes for which personal health information may be collected and used are identified in Appendix B.

4.  The purposes for which personal health information may be disclosed are identified in Appendix C.

5.  The persons authorized to collect, use and disclose personal health information identified in Appendix D.

6.  The persons from whom personal health information may be collected are identified in Appendix E.

7.  The persons to whom personal health information may be disclosed are identified in Appendix F.

8.  The limits and conditions on the collection, storage, use and disclosure of personal health information are identified in Appendix G.

9.  A person whose health information is contained in PLIS may make a disclosure directive as per Appendix H.
