4.  The purposes for which personal health information may be disclosed are identified in Appendix C.

5.  The persons authorized to collect, use and disclose personal health information are identified in Appendix D.

6.  The persons from whom personal health information may be collected identified in Appendix E.

7.  The persons to whom personal health information may be disclosed are identified in Appendix F.

8.  The limits and conditions on the collection, storage, use and disclosure of personal health information are identified in Appendix G.

9.  A person whose personal health information is contained may make a disclosure directive as authorized in Appendix H.

10.  This MO repeals and replaces MO 43/2009, and MO 156/2009.
